Blended Case Management for Children with Mental Health Needs
Blended Case Management offers professional assistance to locate, advocate and coordinate services that address the unique needs of your child for a behavioral health diagnosis.
Blended Case Management:
- Supports families of children, teens and young adults (up to age 21) with autism or a behavioral health diagnosis in navigating complex support systems
- Assists families with needs spanning all areas of the child or adolescent's life, including medical, mental health, educational, social and vocational issues
Programs are administered from the main campus of the Barber National Institute and services will take place in homes, schools and other locations throughout Erie County.
