Blended Case Manager
Support children and adults with autism a behavioral health, or mental health diagnosis in navigating complex support systems.
As a Blended Case Manager, you will offer your unique professional assistance to help find services that will address the varied needs of the client. Supports include, assisting a person's needs spanning medical, mental health, educational, social and vocational issues.
Responsibilities include assisting clients and their families in identifying, accessing and participating in available community resources throughout Erie County, and collaborating with said services.
